When I go to plug my One in, it only will charge if the power is off. If the power is on, it does not recognize the power cord is in.
Is this a hardware failure or software?

Recently my laptop has stopped charging while plugged in. If I unplug the power cord and plug it back in it starts charging again as if nothing was wrong. I'm not certain that it's a software issue, but when I charge it with the power off there are no issues. It also drops from reading 50% to 3% almost instantaneously.
I have the Droid Bionic stock with the original USB cord. I have noticed that while using the cord either with my PC or in my car.. it will not charge my phone. Tonight my phone died and I tried using the cord in the car, and it showed the white light (which I know means phone is under 5% power).
I was wondering if it was possible to have a logging program document a random power failure. Clearly it won't have any time to log when the power fails, but is there a way to recognize a power failure occurred when it reboots after the failure?
I have several problems with my AT&T 8X. First, the screen keeps on cycling through the brightness level non stop, so its constantly changing the brightness. I've tried to turn off the auto-brightness but that doesnt seem to help and this only happen after the phone booted up and I touch the screen so Im thinking it's a software issue.

I have both my computer and monitor plugged into an extension cord and then into a power strip (I've checked, and both the extension cord and the strip can handle the load and have been handling it for a while now).
I used to power off my phone at night while charging when I was using Desire S. Recently I bought HTC One X and can't charge with power off anymore. It keep turning on with the battery and bolt logo and display don't turn off. If I press the power button, the phone is boot again into Android. Now I have to turn on Airplain mode at night when charging.
